Physical exams provided via telemedicine.  

If your employer requires vaccinations or PPD please ensure you have these completed prior to scheduling your appointment as you will need to provide proof prior to the visit.

If your physical form requires vital signs ( blood pressure, heart rate, etc) you will need to obtain these prior to your visit as they will be considered self-reported values.

Cost: $75 via credit or debit card or CashApp.

Payment is due immediately following the visit.  Completed physical forms will be returned to you within 24 hours from receipt of payment.
